What's unique about this book report form?
It includes the opportunity to give the book a Philippians 4:8 rating. Additionally, it encourages the student to turn their answers into a book review and share it with the world via Amazon or Goodreads.

That is a good idea. Book reports are not easy, especially for kids. I would have loved something like that when I was in school
